The first and last name is one of the basic identifiers used by the company. If your surname changes, most often due to marriage, be prepared that pleasant memories of dowries will be replaced by running around the authorities.

(For simplicity, we assume a woman's surname change due to marriage, although of course this is not the only reason why and how a surname can change.)

The form of the surname according to the Civil Code

What are the rules for the form of the surname according to the new Civil Code (NOZ)? It remains the case that it is not possible to use a triple surname. If you already have two surnames and you are engaged, you can only choose the first one as the connecting surname.

Example: If Mrs. Drábková Jenčková marries Mr. Pleskot, her name will be Pleskotová Drábková after marriage. He cannot keep all three surnames.

In the case of divorce, it used to be the case that the divorced spouse could take back his former surname within one month or stop using the joint surname in addition to the former's surname. NOZ extends this period to six months. In the event that the husband has an appended surname, he will adopt it in the form before the marriage.

Since January, it has also been valid that if the adopted child does not agree to take the adopter's surname, he can decide to keep his surname and add the adopter's surname to it.

The journey to your new surname begins before the wedding. Even before the ceremony, you have to submit a so-called Marriage Questionnaire to the marriage registry where you state the surname you will use after the wedding.

Your child's birth certificate

If you are getting married and have children, expect to hand in their birth certificates before the ceremony. In those, the mother's name will change.

Example: Jarmila Květáková will no longer be listed in the child's birth certificate. After you receive the new version, it will already contain the new surname, i.e. Jarmila Hlávková (née Květáková).

New ID card

For a new ID card, go to the municipal office or municipality of your city. There is no fee for this change (unless you want a card with a chip, the issuance of which costs CZK 500). You only have to lend the clerk your current identity card for a moment, on which she will cut off the corner and attach a note confirming the change and new surname to it.

You will not need a passport photo with you for the new "citizenship". The clerk will take a picture of you right when you apply. You can now also choose whether or not to have your marital status entered in your identity card.

The office will issue you a new identity card within 30 days. In most cases, when applying for a new OP, it is already possible to agree that the clerk will send you an SMS that you should pick up the new document.

New driver's license

You apply for a new driver's license using the appropriate form. In addition to your ID card and original "driver's license", you must also have one passport photo with you (as a rule, your birth number is written on the back).

New technical license

Your name includes both a large and a small technical license. He makes the change at the vehicle register. In addition to technical licenses, you must also bring your ID card (or change card if you don't have a new ID card yet).

The change costs CZK 50 for one vehicle and one data. If you were to change your permanent residence, for example, you would pay CZK 100 for one car.

New passport

If you have a passport with your original surname, it will be valid for three months after the date of your marriage. Therefore, when you come to the travel documents department to apply for a new one (according to the information of the passport employee at the Jablonec nad Nisou City Hall, no application is completed), you do not need to hand in the original document right away if you are planning to travel abroad.

You must bring your ID card and 600 CZK to pay the fee. You do not need to bring a photo, the clerk will take a picture right on the spot.

Although you can apply for a new citizen's card at any office, you can only apply for a passport at the one under which you fall under your permanent residence.

New health insurance card

Some health insurance companies are connected to the population register. Therefore, they should receive information about your new surname automatically.

Even in this case, however, it is advisable to confirm the change at least over the phone, as this will probably speed up the process of producing a new health insurance card.

Elsewhere, you need to go to a branch or at least scan a freely worded notice of a new surname with a handwritten signature and the indicated birth number. The insurance company can send the card by post. They can also issue a paper certificate with the new surname as a temporary document. If you do not hand in the original document at the branch, it is possible to invalidate it at home after receiving a new one.

Where should you report the change?

OSSZ/MSSZ/PSSZ

The district social security administrations (municipal in Brno and Prague in Prague) are connected to the register of residents, so they should register the change automatically. However, as we learned from a worker at the OSSZ, the transfer of information does not always work reliably, so in your own interest, you should better check that the information is up-to-date.

Financial Office

The Financial Office is connected to the register of residents, so it should automatically register a change in your surname, as well as, for example, permanent residence. It is only up to you to report the address for mail delivery here at the registration department if it is different from your permanent residence.

Commercial office

The commercial office is also connected to the register of residents, so it will receive a notice about the change of surname from there. It is therefore up to you to go to the office for a Trade Authorization Certificate, which has been issued since 2008 in the form of an extract from the trade register. You pay CZK 20 for each page.

Post

The Czech Post is guided by what is in your mailbox when delivering. It is therefore advisable to have both names on the mailbox for some time, before all delivery providers register this information.

If you still receive a shipment under your original name and you already have a new ID card, you must bring a document that shows that your surname has changed due to marriage, i.e. a marriage certificate.

If you have a SIPO in your name, you must go to the post office and change the information there. The connection number will remain, but you need to know it to change the details, so the best solution is to take one of the SIPO statements with you.

Real estate cadastre

You can solve the real estate cadastre from the comfort of your home. On the cuzk.cz website, it is enough to download the Notice of change of data on the owner or other person authorized to register in the real estate cadastre.

Banks

You must also report the change to your bank. For this purpose, visit the branch with a new ID card. After registering your new surname in the system, you can directly apply for a new payment card here. Most bank clients will also have to create a new signature template at the branch. However, this does not apply to all banks. For example, Equa Bank does not use a signature pattern at all.

Insurance companies

If you have liability insurance, supplementary pension insurance, liability insurance, life or property insurance, etc., you will not miss a visit to an insurance company branch.

This is because it is necessary to make a technical change to the contracts (this is not a reason for a new contract or an amendment). You need a new ID card and contract number with you. It will be ideal if you take a statement or a copy of the contract with you.

Tram ticket

Don't forget to change your surname on your so-called tram ticket, i.e. on your transport card, which is often used today in the form of a loaded card. For example, residents of the capital can apply for an Opencard replacement due to a change of surname at the customer center in the Škoda Palace or online. But only if they have their tram ticket registered.

To exchange a card, you need to present a new identity card, one ID photo (if you did not agree to keep the photo when obtaining an Opencard) and the original card.

You pay a fee for the exchange, the amount of which varies depending on how long you want the exchange to last. You pay CZK 100 for an exchange within 14 days with personal collection, CZK 150 for sending by post (it cannot be sent by post if you requested the exchange online) and CZK 250 for express delivery (only in the customer center).

Energy suppliers

In the case of energy suppliers, the procedure for changing the surname is not uniform (at least according to what we learned on the customer line). According to the operator of ČEZ, it is enough to report the change by phone or send it by email. You only need to know your customer number. The employee of E.ON said that it is necessary to send a copy of the marriage certificate by email or post and also include some numerical data for verification in the message, again for example the customer number or the number of the place of consumption, which is written in your name.

Mobile operators

You must also report the change of your surname to mobile operators. Even in this case, you will not miss a visit to the branch. You must have a new ID card with you.

Loyalty cards

Loyalty cards are usually not difficult to change. Either you just need to log into the internet application, where you can simply change the data, or you can usually find an e-mail on the website, where you can send a request for a change and possibly sending a new card.
