You do not lie to the child before visiting the dentist. And promise him a reward, says a doctor<

How to prepare a child to visit a dentist? Will the treatment always on the first attempt? And when should parents oversee oral hygiene of children? Not only these questions are answered byKamila Hejková, a specialist dentist of the Prague Dental Center Ořechovka.

When to go with the child for the first time to the dentist? And at how many years of age usually parents come for the first time?

The first visit to the child at the dentist should be planned for the period of pruning the first teeth. Thus, the parents usually come to an initial inspection with a child between the sixth and twelfth month of the child's age.On the first visit, it is important to explain to parents that they have to clean the child's teeth right after pruning. It is also advisable to acquaint parents with inappropriate eating habits that threaten baby teeth. At the same time, they should be acquainted with how often they should go with a child for inspections and should be explained to them the importance of these dentures. In the Ořechovka dental center we recommend regular preventive inspections with a frequency twice a year. These inspections are important for early detection of caries, developmental defects and of course also bite defects. In addition, they are important for establishing a smooth relationship of the child to the dentist and the environment of the dental office.

In addition, parents should of course come up with the child also when the child complains about some pain in the mouth or if the parents defend the child's teeth or the gum or oral mucosa something suspicious. It should also be a matter of course from pruning permanent teeth that parents regularly visit with a child's office of a dental hygienist.

Do you prefer a common “family” dentist or special pediatrician?

Today, the trend is the so -called family dentists who treat the whole family, know the habits of its members in the area of dental hygiene and provide all comprehensive care. There is also a frequent model where the whole family is in the care of the dental center, where one of the dentists directly specializes in the treatment of children.Otherwise, there are not many dentists who only treat only children. And every modern dental office should be equipped today for adult care and children's care.Of course there should also be in children making diagnostic X -ray images if necessary. It should also be common for parents to be offered for children and milk teeth for children. In addition, if a dentist also works in children with magnifying technology (magnifying glass or microscope), treatment is always better quality.

How does it be appropriate to prepare a child to visit a dentist to make everything "smoothly"?

Parents should describe the child what to expect at the dentist, what will happen there. As for the child only for a tour and his parents also tell him in advance that it will only be browsing tooths, then it is not advisable to change the plan and perform some intervention. If there is a planned procedure, eg.Driving of caries and making a filler, it is advisable for parents to describe what will be done to the child. We have proved to be successful if the child is watching one of the parents before the inspection. The child sees how important it is to have an open mouth and work together. Before the procedure, it also does not hurt at all when parents promise a child for good cooperation some reward. It is also important to choose a suitable daily time to treat so that the child is not drowsy or annoying.

Do you meet in your practice with children who do not want to be treated, crying, afraid, do not want to open their mouths and the like? How do you proceed in such cases? Will all children manage to treat on the first attempt, or have you ever approached analgosedation?

It should be emphasized that during the child's stay in the dental office are very important to the dentist's accessible attitude and the overall pleasant atmosphere. The child should feel good in the dental office, and right on the first visit.Of course there should be praise of the child for proper tooth cleaning and some attention (picture) for good cooperation in treatment.Of course, such cooperation of the child during examination or treatment is always individual according to the age of the child, but also the nature of the child, or possibly also his earlier experience with doctors. If the child does not cooperate, the dentist should be patient or try the treatment on the next visit (for example at another time of day) after the previous psychological preparation from parents.

If the child is untreated and the treatment of caries is necessary, it is necessary to perform treatment under general anesthesia or in analgosedation. The fact that small children do not want to be treated is not exceptional. The individual approach of the doctor is then important. Some child pays gentle persuasion, others even a more vigorous word and a strong challenge to open the mouth.Great importance is in these situations the assistance of parents who mostly know what their child pays. The treatment is not always possible at first attempt, but an experienced dentist usually recognizes whether the attempt is another or the child is really untreated without analgosedation or general anesthesia.

What are the initial interventions in your child patients?

Of course, the most common is the treatment of tooth decays with fillings.Today, in almost all cases, we make white fillings in children, which in children's teeth are much better and we do not have to drill our teeth as for them for amalgam fillings.Another frequent procedure in children is tearing (extraction) of temporary teeth due to too large caries or also because of their swinging before replacement for a steady tooth.

How many years was your youngest patient you have treated tooth decay? Are tooth decay common in children? How would you evaluate the condition of your child patients' teeth?

The youngest children we treat caries are about the age of three years old.Children who are under three years of age will be very difficult to persuade to cooperate and is definitely better if the caries do not arise at all.Unfortunately, tooth decay in children is very common today and it is definitely involved in drinking a large number of sweet beverages and insufficient supervision of dental hygiene of children by parents. The teeth of my pediatric patients is really different and it should be emphasized that children depend mainly on parents who allow children to consume and how to clean their teeth.Parents have to realize that small children simply do not clean their teeth themselves.

What do parents have in general care for children's teeth, reserves?

In the level of dental prevention in children, including awareness of the importance of dental hygiene.Parents often come for the first tour only at a time when the child already has the first caries. Not all parents also realize that even children should regularly occur to a dental hygienist where the child will acquire the right habits of care for their teeth. It is clearly proven that individual training tooth cleaning is the best method for caring for caries in children.Care for child (milk) teeth is important and there is a risk of the correct development of permanent teeth when neglecting.Spoiled front teeth can affect the psyche of the child and premature loss of temporary teeth can affect the correct pronunciation in children.Premature loss of milk teeth can also lead to bite defects.Parents should definitely realize eg.great harm of sweetened beverages and especially that for teeth is the worst long -term contact with sweet, ie constant sipping these drinks or eg.sucking.As has already been said, the second thing that parents must know is that a small child does not clean their teeth properly and parents should teeth after it.

What is your opinion on the practice of previous years - dental inspections within the school?

School tours at the dentist had the advantage that the dentist also got children who today's parents bring to the dental office only when some tooth hurts. However, the big disadvantage of these examinations was the non -individual's approach to children, non -cooperation with their parents and definitely lack of dentist's time for each individual child.

During today's tours, it is time to discuss with the parent eating habits, the method of teeth cleaning and if there is a problem in the child, it is possible to discuss the possibilities of solving and eventual benefits eg.different types of dental fillings.

How to clean your child properly?

The basis is to use the right toothbrush twice a day.The brush should have a small head and soft, straight cut bristles. We choose a brush to keep the child well in hand and not too massive for a small meringue. The amount of toothpaste should be small in young children, in the size of peas, in larger children the amount of paste is the same as in adults.

Even in children we must not forget to clean the interdental spaces, ie places that are susceptible to tooth decay. We will start to emphasize this cleaning of interdental spaces mainly at age when children already have permanent teeth in their mouths. The technique of proper tooth cleaning - circular movements - is trained with children and parents by a dental hygienist who explains to parents how to read the teeth. With their parents they also practice the use of interdental aids - floss and interdental brushes.Using a floss with outstretched and prepared thread on a plastic sling is easier in pediatric patients than cleaning with ordinary thread. With interdental brushes we start immediately after pruning permanent stools, which is around the age of six.The popular additional aids include Solo brush. It is used without toothpaste and is used to clean the stools, which children get poorly with a classic brush.Solo brush also greatly appreciate parents to clean their children's teeth.

Solo brush is also called one -volume or single brush. The brush has a very small head, consisting of one bundle of shorter bristles made of fine fibers.

When should children brush their teeth for children and when is it time for the child to become independent?

Until a certain age it is necessary to clean the teeth for children, it always depends on the age and skill of the pediatric patient.Parents' supervision when brushing children's teeth is very important.Children do not yet have such fine motor skills and there are places where they get bad with the brush. Therefore, it is necessary for parents to receive these poorly cleanable places for children.Generally, we recommend parents to check and possibly clean their teeth under ten years of age.The longer dental hygiene they check in children, eventually correct the better, the better.Parents should oversee adherence to dental hygiene even during adolescence, when children often neglect tooth brushing and often start to develop a large number of interdental caries in juveniles.Parents should lead their children to regular examinations at the dentist and also to regular visits to the dental hygienist.

Can you give some interesting things, a practice case, a "pearl", or some recommendations concerning this issue?

Interesting is what toys sometimes children come to the dental office.Sometimes they bring their favorite stuffed stuff with them and want the stuffed toys to watch or participate in it in some way.It is also not exceptional that children want the other family members, including all siblings.Other children do not want anyone else to look at the tour. The exception is not even inquisitive children who want to explain what different dental tools are.A dentist nursing children must be patient and must be prepared for often surprising children's reactions.
